Community Health Worker - REVISED

Description of Duties
This is a community outreach position in the Health Department responsible for making home visits to increase health care awareness in areas such as pediatric care, among others. The employee provides families and individuals with basic information regarding health care and assists them in obtaining health care services provided by the Health Department. Work may include nights and weekends. The employee reports directly to, and works under the general supervision of, a higher level employee. Supervision is not a responsibility of this class. Does related work as required.
Minimum Qualifications
Graduation from high school or possession of an equivalency diploma, plus two (2) years paid full-time or its part-time or volunteer equivalent supervised* experience working with young children** and/or parents such as day care providers or teachers.
*Unsupervised experience such as private babysitting is not qualifying experience.
**For the purpose of these minimum qualifications young children is defined as children under five (5) years of age.
