Description of Duties
This is a professional nursing position located in a nursing unit at Monroe Community Hospital responsible for providing case management intervention to ensure optimum health care and positive clinical outcomes over a continuum of time and cost efficiency of health care services. Duties involve performing utilization review using case management software applications that provide pertinent data from a variety of sources of resident information (i.e. claims, laboratory results, medications, health risk assessments) in order to avoid the under or over-utilization of health care leading to lower quality care and higher costs and health risks. Duties also involve completing and monitoring the patient assessment process. The employee reports directly to, and works under the general supervision of a Nurse Manager or other higher-level staff member. General supervision is exercised over paraprofessional and clerical staff.
Minimum Qualifications
Graduation from high school or possession of an equivalency diploma plus graduation from a regionally accredited or New York State registered college or university with a Bachelor’s or Associate’s degree in Nursing and eighteen (18) months paid full-time or its part-time equivalent* professional nursing experience in a skilled nursing facility.
